Apple’s Notes recorder doesn’t a pretty good job. I’ve used it on number of occasions from keeping notes in notes to jotting ideas on the move.

All it needs is the NLP side to understand intent, context and action.

A startup would find it challenging to compete with these free tools; even the one from google (translate). It feels more like a feature or app in the EHR marketplace suite.
